A commitment to lifelong wellness and safety will be actively promoted through classroom and district-wide health screenings, and educational and training programs each school year. We ask for your cooperation in providing us with a current health history of your child as it better ensures a safe transition from home to school.

Health History
We ask that the Health Information section in Skyward be completed by all parents/guardians when registering their child within the Johnson Creek School District. If you list any health concerns which may include asthma, severe allergy, diabetes, seizures, migraines, and/or any medication(s) that will need to be administered at school, you will also need to complete correlating forms listed below. Please print and have them on hand for your child's health visits, as they may require a health practitioner's signature. Please return all necessary forms to your child’s school office or the health services office on or before the first day of school to ensure a safe transition from home to district.
I review all health histories of children attending our Johnson Creek schools. I then share only necessary information and train appropriate staff to better care for your child’s individual needs during the school day. Please make sure to communicate any new health findings/changes, as well as any changes to your EMERGENCY CONTACT information to the school or health services' office throughout the school year.

Physical Examinations
A physical examination is required for all students entering 4K and 5K. It is also recommended that they receive an eye and dental exam prior to enrollment. If your child at any other grade level would also complete an annual physical, eye, and/or dental exam, please find the correlating forms below. You will need to print and take these forms to your child’s appointments for the medical provider(s) to complete and provide a signature.

By Wisconsin state law, the school is responsible for assuring that all student immunizations are up to date or that a signed waiver is on record for all children within our school buildings. Each parent is responsible for notifying the school in writing of immunizations received. See the immunization requirements for each grade level, and the STUDENT IMMUNIZATION RECORD below, to take with you to your student wellness check/vaccination appointment.
