School Nurse

We ask that the Health Information section in Skyward be completed by all parents/guardians when registering their child within the Johnson Creek School District. If you list any health concerns which may include asthma, severe allergy, diabetes, seizures, migraines, and/or any medication(s) that will need to be administered at school, you will also need to complete correlating forms listed below.  Please print and have them on hand for your child's health visits, as they may require a health practitioner's signature.  Please return all necessary forms to your child’s school office or the health services office on or before the first day of school to ensure a safe transition from home to district.

I review all health histories of children attending our Johnson Creek schools. I then share only necessary information and train appropriate staff to better care for your child’s individual needs during the school day. Please make sure to communicate any new health findings/changes, as well as any changes to your EMERGENCY CONTACT information to the school or health services' office throughout the school year.

A physical examination is required for all students entering 4K and 5K. It is also recommended that they receive an eye and dental exam prior to enrollment. If your child at any other grade level would also complete an annual physical, eye, and/or dental exam, please find the correlating forms below.  You will need to print and take these forms to your child’s appointments for the medical provider(s) to complete and provide a signature.

By Wisconsin state law, the school is responsible for assuring that all student immunizations are up to date or that a signed waiver is on record for all children within our school buildings. Each parent is responsible for notifying the school in writing of immunizations received. Illness Policy

Sick kids belong at home and well kids belong at school!
If your child is ill, we ask that they stay home to limit exposure to other children. Because it can be difficult to determine if your child is too ill to attend school, please refer to our Illness Guidelines as a tool, understanding that medical advice should be asked of your child’s health care practitioner.

Because Covid-19 is still present in our community, guidelines and exclusions from school will be updated and communicated as needed.

The Johnson Creek School District follows a Medication Administration Practice for trained staff to safely administer practitioner-ordered and as needed medication(s) to students during the school day. Please see the Student Handbook to view the practice in its entirety. Highlights are listed below:

• All prescription and non-prescription medication given at school will need a completed and signed PARENT/PHYSICIAN CONSENT FORM FOR MEDICATION ADMINISTRATION on file for the current school year
• All medication will be brought into the school by a parent/guardian, not the student
• All prescription medication requires a prescribing practitioner’s signature and a parent’s signature and will be kept in a locked cabinet in the health services' office
• All over the counter (OTC) medication requires a parent’s signature, and administration will not exceed the recommended dosing on the package
• All prescription and OTC medication will be brought to school in its original manufacturer’s packaging or pharmacy dispensed container
• All students needing medication will have it administered in the health services' office
• All emergency medications (inhalers, EPI pens) can be carried by the student with a physician’s signed and stated directive. Please include the medication administration form and the severe allergy form for emergency planning
• Any medication changes, dose changes, or discontinuation of a prescription medication, must be verified in writing by the prescribing practitioner
• No medication will be administered to students without the proper forms completed and on file in the health services office.
